Unable to upload avatar, I've tried all sizes and file types - all come back with "The file could not be uploaded."

This should be fixed now, let me know if you run into any further issues with it! Similarly, there might be some other image uploads that don't work (that should). If you spot any more please let me know
There was a permissions issue that's different in the test/live environment.

Thanks Rhu, I confirmed I was able to update my profile image and have updated this to CLOSED.

Changing your profile background image works nicely too!
